Talent Release and Authorization

I, the undersigned, hereby consent to the audio recording and/or videotaping of myself, or on behalf of my minor child, for commercial and/or promotional purposes. I authorize Edit House Productions, LLC and the Client / Business named below, its agents or assigns, to cause video and/or still images or audio recordings of myself and/or my minor child to be exhibited on broadcast television or radio, on the World Wide Web, or in any print medium, in perpetuity.

This agreement is a complete release with no monetary exchange for talent services. I understand that I will not be compensated by Edit House Productions, LLC or the Client / Business named below. Additionally, no residuals or additional fees will be paid in the future.

By signing this form, I release Edit House Productions, LLC and the Client / Business named below, its employees, advisors, consultants, agents, or assigns from any and all liability and claims for monetary damages for libel, slander, invasion of the right of privacy, or any other such claims based on the use of the above-described material.
